This paper shows the practice problem HCC patients face and how screening tests can be used to find HCC early at the gastroenterology clinic using a practice gap assessment tool. The PICOT framework is also used to determine the organization's practice question. The paper also speaks about how to lead the practice change initiative to solve the problem that was found. Finally, the paper explains how a leader may effect change in a team by being innovative and efficient in their management style. Organizational Needs Assessment Anorganizational needs assessmentis a process that gives an organization a way o evaluate skills, knowledge, or practices. The paper aims to help an organization determine what might be holding it back from reaching its goals. A needs assessment helps a group determine what is going well and what needs to be changed. Problem According to Trung et al. (2018), hepatocellular cancer (HCC) is one of the most aggressive forms of cancer and the third most significant cause of cancer-related mortality globally. Although HCC treatment has come a long way, it has been recognized for a long time that the outcomes for HCC patients still require improvement. The high death rate from HCC is also because it is not found early (Zheng et al., 2018). In order to help with the early detection of HCC in the specific gastroenterology clinic, the key stakeholders include patients, healthy volunteers, family members, providers, organization leaders, clinical executives, quality and safety specialists, and regional and national leaders. In addition, the gastroenterologist and the nurse practitioner are both members of the practice organization. Practice Gap Practice gaps are the discrepancies between what the clinic knows and does currently and what it should know and do. Finding the cause of the gap, determining the organization's need, choosing the best evidence-based approach based on that need, and putting evaluation and assessment techniques into practice all benefit from a gap analysis. According to the Will County Health Department survey (2021), between 2015 and 2019, there were 63 females and 158 males diagnosed with liver cancer. Patients and payers have a heavy financial burden associated with HCC monitoring and therapies, which cost more than $500 million annually in the US. The current practice at the clinic is the combination of ultrasound and serum alpha-fetoprotein (AFP) for close monitoring and detecting the presence of HCC in high-risk groups. Understanding imaging data might be complex for some patients. AFP's low sensitivity and specificity require the development of biomarkers with improved sensitivity and specificity for HCC diagnosis
