HPI: The patient, D.A., is a 17-year-old Hispanic female presenting with panic features and depression to the clinic. The patient does not report how long she has been experiencing the symptoms. She reports taking her medications every day, including Abilify, Pristiq, and Zoloft. The patient reports having suicidal thoughts and feelings of worthlessness and helplessness. Additionally, she reports that she has been unable to do her school work and has been feeling soanxious. She describes her anxiety as horrible and off the charts. D.A. also reports hearing voices and feels it is telling her she should kill herself and at times talks back to the voices and believes in what they are telling her. However, she reports feeling safe at home because her family keeps a very close eye on her. Past Psychiatric History: General Statement: The patient is a 17-year-old Hispanic reporting for treatment of depression and anxiety. Her reported symptoms include anxiety, depression, panic, and suicidal thoughts and ideation. Caregivers: D.A. reports that her family keeps a very close eye on her. Additionally, she reportsthat her uncle, Boris, is available to keep her safe from sharp objects and medications and monitor her. Hospitalizations: The patient denies any recent or past hospitalizationsMedication trials: The patient is also negative for any medication trialsPsychotherapy or Previous Psychiatric Diagnosis: The patient has not been undergoing any form of psychotherapy and has not been treated for or diagnosed with psychiatric disorders prior to this visit
