reason for care: Chest pain.
Present illness: Previously healthy patient woke up in 4 A.M. with the retrosternal chest pain. The pain was described as squeezing, crushing and burning, scoring the pain as 7/10, with radiation to the shoulder, neck, jaw and left arm.
Perception of health: Prior to chest pain, the patient states that his health was good but he does not exercises regularly and his diet is not “healthy”.
Medical history: Patient has diagnosis of essential hypertension and hyperlipidemia. He was diagnosed in 2010. Medication that he is taking since are perindorpil and indapamide (stating that both medications are taken once a day, usually in the morning). He is not allergic to anything. He is regular with vaccinations and immunizations. He denies any operations or major infections.
Family medical history: Both of the patient’s parents had high blood pressure. Patient’s father died from a heart attack when he was 62 and mother died from untreated pneumonia when she was 70.
Review of systems: He had no significant weight gain or loss. In general, patient is feeling weak and fatigued. The patient denies loss of vision, blurred or double vision, flashing spots or lights, dryness and pain in the eyes. He denies loss of hearing, ringing in the ears and ear pain. He denies loss of sense of smell, nasal
